3. CONSENT TO PARTICIPATE

I, the undersigned parent/guardian, give permission for my child to participate in the Kids Coding & Robotics Training Program organized by BREATH OF HEAVEN ACADEMY.

• The program involves computer-based learning activities including coding, basic software development and digital literacy.
• My child is expected to follow all class rules and instructions provided by trainers.

4. HEALTH & SAFETY AGREEMENT

• In case of minor illness or injury, basic first aid may be administered.
• In case of emergency, the company may seek medical attention for my child, and I will be contacted immediately.

5. LIABILITY WAIVER

• BREATH OF HEAVEN will take all reasonable precautions to ensure the safety of my child.
• I agree not to hold the Company, its staff, or trainers liable for any injuries, loss, or damage that may occur during the course of the program, except in cases of gross negligence.

6. CODE OF CONDUCT

• My child will behave respectfully toward trainers and fellow students.
• Any form of misconduct (bullying, disruption, misuse of equipment) may result in disciplinary action, including removal from the program.
• No refund will be issued in cases of dismissal due to misconduct.

7. USE OF EQUIPMENT

• My child is responsible for any personal computer or device brought to class.
• BREATH OF HEAVEN ACADEMY is not liable for loss, theft or damage of personal property.

8. PAYMENT POLICY

• I acknowledge that all fees paid are non-refundable once the program has commenced.
• Missed classes will not be refunded or rescheduled unless stated otherwise.
